I hereby grant permission for my child named above, to attend Crossings Student Camp at Cedarmore, May 31, 2025- June 3, 2026, at Cedarmore Camp and Conference Center, 3083 Cedarmore Rd, Bagdad, KY 40003
I grant permission for the representative of First Baptist Church of Belfry to take my child to the nearest medical facility and receive appropriate medical care should an accident or unforeseen event resulting in injury occur.
I understand that should my child misbehave; I may be asked to transport my child from the camp in which he/she is participating. This will only occur in an extreme circumstance.
I will be responsible for any medical bills resulting in my child’s injury or illness beyond the control of the chaperone.
I understand and grant my full consent to all the above.
I hereby authorize a representative of First Baptist Church of Belfry to transport my child to and from camp during the dates of May 31, 2026 -Jun3 3, 2026. I understand that by allowing a representative of First Baptist Church of Belfry to transport my child to/from above listed camp, I agree to release First Baptist Church of Belfry and the representative of First Baptist Church of Belfry from and against any and all liability, loss, damages, claims, or actions to the maximum extent permissible by law, arising out of such transportation.
I have read, understand, and discussed with my child that:
*They will be traveling in a motor vehicle driven by an adult and they are to wear their seat belt while traveling.
*They are expected to respect each other, the vehicles they ride in, and the people they travel with during the trip.
*They are to remain in their seats and not be disruptive to the driver of the vehicle.
We will not release you child to anyone except you unless prior notice is made known by parent.
